Mind and Body Wellness provides specialized Psychiatric Nexus Evaluations for Illinois veterans. The clinic understands the complexities of VA disability claims and provides the rigorous clinical documentation needed to support these cases.
A psychiatric nexus letter is a professional medical opinion used to support a veteran’s VA disability claim. The letter explains whether a diagnosed mental health condition is connected to military service or has been caused or worsened by another service-connected condition.
Nexus letters are commonly used when veterans are filing an initial claim, appealing a denied claim, or seeking additional medical evidence to support a case. A well-prepared nexus opinion includes a clinical evaluation, a review of relevant records, and a clear explanation of the medical reasoning behind the opinion.
Psychiatric nexus evaluations may address conditions such as:
PTSD (Post-Traumatic Stress Disorder) & MST (Military Sexual Trauma)
Depressive Disorders (Major Depressive Disorder, Dysthymia)
Anxiety Disorders (Generalized Anxiety Disorder, Panic Disorder)
Sleep & Trauma (Insomnia, Trauma-related disorders)
Adjustment Disorder (Related to service or transition)
Secondary Service Connections (e.g., Depression secondary to chronic pain or other service-connected physical injuries)
A psychiatric nexus evaluation may be helpful for veterans who:
Are filing a VA disability claim for a mental health condition.
Have had a claim denied and need additional medical support.
Need documentation connecting PTSD, depression, or anxiety to service-related experiences.
Have a psychiatric condition that may be secondary to another service-connected medical condition.
The psychiatric nexus evaluation process typically includes:
1. Comprehensive Psychiatric Evaluation: A dedicated clinical interview.
2. Record Review: A thorough analysis of available military and medical records.
3. Clinical Assessment: Evaluation of symptoms, diagnosis, and functional impact.
4. Medical Opinion: A written medical opinion provided when clinically appropriate.
A nexus letter is not automatically guaranteed following an evaluation. The medical opinion must be supported by the clinical interview, the history provided, and the records reviewed. The opinion provided is based on the information available at the time of the evaluation; additional records not available during the assessment may affect the final medical opinion.
Veterans are encouraged to provide relevant records when available, including:
VA Rating Decisions
Prior Psychiatric Evaluations
Service Treatment Records (STRs)
Compensation and Pension (C&P) Examination Reports
Psychiatric Nexus Evaluation & Medical Opinion: $900
To maintain clinical independence and provide the specialized time required for these forensic reviews, Nexus evaluations are private pay services and are not billed to insurance. Payment is required in full prior to scheduling the evaluation.
This flat-fee service includes:
Comprehensive Psychiatric Evaluation: A diagnostic clinical interview.
Detailed Record Review: Thorough analysis of VA rating decisions, service treatment records, and prior psychiatric evaluations.
Professional Nexus Letter: Preparation of a formal medical opinion using VA-compliant language when clinically supported by findings.
What exactly is a "Nexus"? A nexus is the medical link between a current diagnosis and military service. The role of the evaluator is to determine if that link exists and document it using the VA-required language of "at least as likely as not."
Is an evaluation necessary if a diagnosis already exists? Yes. Even if a diagnosis has been made by the VA or another provider, a Nexus Evaluation is specifically designed to review service records and provide the formal medical reasoning needed to connect that diagnosis to military service.
How long does the process take? Once all relevant records are received and the evaluation is complete, letters are typically finalized within 7–14 business days.
Is a positive nexus letter guaranteed? No. A psychiatric nexus letter can only be provided when the clinical evidence and a thorough review of records support a service connection. Mind and Body Wellness must base all opinions on clinical findings. If the evidence does not support a service connection, a nexus letter will not be issued; however, the veteran will still receive a full clinical evaluation.
Why is this "Private Pay" only? Insurance companies generally do not cover medical-legal opinions or forensic evaluations used for disability claims. Private pay ensures that the evaluation is independent and thorough.
Mind and Body Wellness approaches psychiatric nexus evaluations with the same care and professionalism brought to all mental health services. Evaluations are thoughtful, thorough, and grounded in clinical experience. The clinic understands that a nexus evaluation is a vital step in the VA disability process and takes the time to carefully review history, symptoms, and available records.
The goal is to provide clear, well-documented psychiatric assessments that accurately reflect a veteran’s mental health history and current condition. Every evaluation is conducted with respect for the experiences of those who have served and with attention to the details that matter when preparing a medical opinion.
